1. CONSENT. Completing this registration form you hereby consent to participate in the production of media which may be used to show my image, likeness, voice, performance, and visual works which may be personally identifiable to the general public (“Media”) 

                       a.) Media Types. Consent is granted to publish the Media in any format, including 

                         photographs, video recordings, audio recordings, and digital recordings. 

1. RELEASE. After completing this form you hereby comply to(“Released Party”) its affiliates, successors, and assigns the irrevocable, worldwide, and perpetual right to use of the Media for personal or commercial use. 

                       a.) Right to Edit. The Released Party shall have the right to edit, alter, adapt, or modify the 

                         Media and any related or unrelated materials. 

1. COMPENSATION. I, (“Consenter”) understand that there will be NO compensation in return for my consent and I agree to this.  

1. RELEASE & WAIVER. I, (“Consenter”) hereby release, discharge, and hold harmless the Released Party and its representatives from any and all claims, demands or causes of action that I may have, including, but not limited to, claims based upon defamation, invasion of privacy, rights of publicity, or injury arising from the use of the materials as described herein and/or by the general event. 

1. DISCLAIMER. Past performance may not be indicative of future results. Participation in the Human Life Social Activities (HLSA) does not guarantee any specific outcomes or life changes. All growth is personal, and results will vary based on individual effort, circumstances, and choices. Therefore, you should not assume that the experiences or progress of others  regarding the HLSA will reflect your own. By attending, I, (“Consenter”) acknowledge that I am solely responsible for how I apply what I learn and that no promises or guarantees have been made regarding my future results. 

1. REPRESENTATIONS & WARRANTIES. I, (“Consenter”) represent and warrant that I am or a parent/guardian is at least 18 years of age and have the full legal capacity to execute this release. The rights I am granting do not conflict with any existing commitments or agreements, and I have read and understand the contents of this release. 

(By completing this registration, you are agreeing to every section of this document.) A legal parent/guardian will have to sign for participants under the age of 18.
